Employee Benefits

 

  • Medical Insurance: Four PPO plans offered through Humana and are available to regular full-time and regular part-time (at least 20 hours per week) employees on the first day of the month after 30 days of employment. Coverage categories are defined as Employee, Employee plus Spouse, Employee plus Spouse, Employee plus Child(ren) and Family. Domestic partner coverage is also available.

     
  • Dental Insurance: Three types of dental plans offered are with the following providers: Delta Dental (Premier and Preferred) and CompBenefits (CompDent AVK1). These plans are available to regular full-time and regular part-time employees. Coverage begins the first day of the month after 30 days of employment. Coverage categories are defined as Employee, Employee plus Spouse, Employee plus Spouse, Employee plus Child(ren) and Family.
     
  • Vision Insurance: National Vision Administrators (NVA) offers Vision insurance to all regular full-time and regular part-time employees. Coverage begins the first day of the month after 30 days of employment. Coverage categories are defined as Employee, Employee plus Spouse, Employee plus Spouse, Employee plus Child(ren) and Family.
     
  • Retirement plan: U of L Healthcare offers a fully funded defined contribution plan where the employee determines how money is to be invested. Employees may contribute in the 403(b) plan after 1 hour of service. Company contributions of 50% of the employee’s first 4% contribution begin after 1 year of service and at least 1,000 hours worked. Employees become fully vested after 5 or more years of service. The maximum contribution for 2010 is $16,500. If you are 50 years of age or older then you will be eligible to contribute an additional $5,500 for the special election catch up.
     
  • Group Life Insurance/Accidental Death and Group Supplemental Life: One times base annual salary is paid by U of L Healthcare for regular full-time and regular part-time employees. Employees may purchase up to four additional times salary (total of 5 times salary) . Cost is dependent upon age and salary.
     
  • Dependent Life Insurance: Employees can choose from three tiers of coverage; low, high and max. Dependent life coverage covers unmarried children from 14 days to age 19. Full time students who rely on the insured for financial support are covered from age 19 through age 25. This is available to regular full-time and regular part-time employees.
     
  • Long-Term Disability (LTD): The long-term disability benefit pays a portion of salary after the employee has been off work for five months. The monthly benefit is 60% of the pre-disability base annual salary up to a maximum monthly benefit of $8,500 for regular full-time employees. Regular part-time employees may purchase LTD at group rates.
     
  • Cancer Insurance: Employees have the option to enroll themselves and their immediate family in the cancer plan offered by Colonial Cancer. This supplemental insurance will reimburse the employee for routine cancer screening tests or will pay if diagnosed with cancer after the waiting period.
     
  • The Farmington Company has the following plans available for supplemental insurance:

    • Short Term Disability
    • Accident Insurance
    • Critical Illness


*You may apply if you have worked for three months or more and are regularly scheduled for 20 hours or more per week.

  • Flexible Spending Accounts: Employees may contribute between $52 and $3,000 of pre-tax earnings in a healthcare flexible spending account or $52 to $5,000 of pre-tax earnings into a dependent care flexible spending account
     
  • Group Legal Plan: MetLaw is a legal services plan administered by Hyatt Legal, a MetLife company. MetLaw provides employees easy access to professional and competent attorneys at an affordable rate.
     
  • Auto and Home Insurance: Employees may purchase auto and homeowners at group rates through MetLife insurance on an after tax basis. This is offered through payroll deduction.
     
  • Tuition Reimbursement: U of L Healthcare reimburses the employee up to $2,500 per calendar year for regular full-time and $1,250 per calendar year for regular part-time employees. Employees become eligible after completion of 6 months of service.
    Scholarships: A maximum of $25,000 is available per student. For every $5,000 of tuition assistance, participant is required to give UofL Hospital 1 year of employment after completing degree and obtaining license/certification. Books are also paid through the program, amount not to exceed $500 per semester.
    Loan Pay-Back: To receive the loan payback benefit you must apply within 1 year of hire date. Awards will not exceed the amounts for an academic year ($2,000 of actual tuition for full-time employees and $1,000 of actual tuition for part-time employees).Loan payback recipients must remain employees of University Hospital in designated positions to continue receiving monthly loan payback awards. A minimum of a twelve month commitment (after the one year payback) is required for each academic year of loan payback dollars received.
     
  • Shift Differential: 12% Evenings (3p-11p), 15% Nights (11p-7a). Must work 4 hours into shifts and be an hourly employee to be eligible.
     
  • Flex Accruals: Regular full-time and regular part-time employees earn flex time each pay period. The number of hours earned each pay period is determined by the length of service and worked hours. (0-4 yrs = .07309 / 5-9 yrs = .09231 / 10+ yrs. = .11153) X _____ (hours worked in pay period)
     
  • Sick Accruals: Regular full-time and regular part-time employees earn sick time each pay period. The number of hours earned each pay period is determined by worked hours. (.0385 per hour X _____ (hours worked in pay period).
     
  • Bereavement Pay: Granted to regular full-time employees for death of immediate family members. Maximum 3 days or 24 hours pay.
     
  • Jury Duty: All regular full-time employees receive their regular base pay plus pay from the court for the scheduled work days they are on jury duty.
     
  • Adoption Assistance: Available to regular full-time and regular part-time employees. Maximum benefit $2,000, however, employee may be reimbursed up to $3,000 for a special needs child per adoption per year.
     
  • Professional Liability Insurance: Provided to all professional employees.
     
  • Employee Awards: Outstanding Employee, Nurse Excellence and Physician’s Choice awards are presented on a quarterly basis. Quarterly winners are eligible for an annual award. An Annual Service Awards Banquet is held for employees. Perfect Attendance is also presented on a quarterly basis.
     
  • Employee Assistance Program (EAP): EAP Services are provided through The Behavioral Medicine Network. Employees and their dependents are provided with 6 free visits annually.
     
  • To assist employees in need of emergency catastrophic financial assistance, U of L Health Care will donate, upon approval, financial assistance per 12 month period.
  • Employee Discounts: We offer a wide variety of discounts such as: Cafeteria, Employee Pharmacy, Dell Discount, Verizon Wireless, Ticket Master, various gyms around town, and discount tickets to regional entertainment attractions.
     
  • Parking: Free to all employees.
